Return Authorization Form

Before requesting a return authorization # make sure of the following:

  • This request is within three days of order receipt.
  • Item you return is NOT stockings, socks, undergarment, a mask, wig, hat, make-up, hair spray, or other item that is used on the mouth or hair area. These may not be returned. Consult the return policy section of this web site for further information.
  • Merchandise must be unused and in original packaging. No return can be granted if costume has any sign of perspiration, or other stains, has been washed, is torn, or shows any other kind of use. This is to protect you and other Costume Universe future customers from receiving anything but new costumes.
  • Item is NOT a special order or sale item.
  • Item has not been damaged due to shipping problems or any other situations beyond our control. If item is damaged in shipping please contact the shipping carrier. Included in the shipping cost is up to $100 in coverage for shipping damages.

You will receive an e-mail with a Return Authorization Number (RAN). The message will be sent to the e-mail address you provided.
Please print the message and place it inside the package so that we may be able to credit your account.
Please write the RAN number on the outside of the package.

On approval, buyer shall be responsible for shipping costs both ways unless return is due to Costume Universe failing to meet its obligations. Failure to deliver your order as requested is rare but at high volume periods can happen. If you receive an order that was not what you specified we will give you a complete refund. We recommend that, in those cases where you have received the wrong item and you need to exchange quickly, simply reorder and note in the comments that you are needing an exchange for an order where Costume Universe provided the incorrect item. We will expedite shipping and refund you for the original item using this form.

Note: Please make sure you have chosen the correct size. We cannot provide credit for items that do not fit as a result of choosing the wrong size. If the item meets the above criteria we can offer you an in-store credit for items that are either the wrong size or do not meet your needs. Remember, DO NOT WEAR ANY COSTUME YOU BELIEVE WILL NOT FIT OR DOES NOT OTHERWISE MEET WITH YOUR APPROVAL. IF YOU WEAR IT, IT BELONGS TO YOU.
